Billing and Payment Policies

The insurance and billing process can be confusing. Each person's plan can vary from one person to another and have different benefits or exclusions. We work diligently to file your claims efficiently and are always available to answer questions concerning your billed claims.

It is your responsibility to contact your insurance company to verify your provider network. You should be aware of the specific benefits or exclusions of your personal plan. The customer service number for your insurance carrier can be found on your insurance ID card.

After we receive your scheduling and insurance information, Old Moultrie Surgical  Center will contact your insurance carrier to verify eligibility and medical benefits at our center. If a deductible or co-pay is required, you will be asked to render this amount at the time of service. Failure to resolve payment issues prior to surgery could result in the postponement of surgery. We accept VISA, MasterCard, and Discovery payments, cash or check. 

 

If Old Moultrie Surgical center  does not participate in your insurance plan, you may still be able to receive services here if you have "out of network benefits." However, since you insurance company will consider our services as being "out of network" you may be responsible for paying more of the bill.

 

Preparing for your visit

Please bring these items with you to the Ambulatory Surgery Center

  • insurance card

  • valid driver's license

  • payment for co-pay or deductible; and any services that are not covered by your healthcare plan.

 

Self Pay

Self-pay patients are required to pay in full for services prior to said service. Where possible we will estimate the required payment when you schedule your service. Please contact our office directly at 904-797-6627 for price estimates. For financial assistance and charity cases the patient may be directed to the nearest hospital  by a credentialed physician.

Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ambulatory surgery center to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.
